Diabetes Review

For patients who are due an annual Diabetes review.                     

Diabetes is a medication condition that requires reviewing annually to ensure your medication is the correct type and dose to control your condition. This annual review will be with the Practice Nurse.

Even if you are well and your condition has not changed since your last review it is important that you book and attend your review.

The surgery will attempt to contact you a maximum 3 times around the month of your review reminding you to make your appointment.

If you feel unwell at any time, please contact the surgery - please do not wait untill your review is due.

Being a diabetic it is recommended that you also have an annual retinopathy screening check with an optician, you will be sent information from the diabetic retinopathy screening service before you at are due your annual check this information will contain the opticians who take part in the scheme for you to choose where to book an appointment this information will also include the pin number that you will need when booking the appointment.

You should also have an annual foot screening check the community Podiatrist currently visits the surgery on the 1st & 3rd Mondays each month 9.15 - 11.30am the practice will book you an appointment and send you notification of this at least 2-3 weeks before the appointment date.
